Employee Evaluation Form

This year we have decided to simplify our “Employee Evaluation Form”. The old form was just too complicated and time consuming not to mention the process of gathering results. So over the span of 30 minutes and a 24 pack we put together what we feel is a more conscious form as to what really matters when evaluating an employee.
